Supporting document of name changing application due to the bad meaning of the name or for any other reason required by islamic law. At first, applicant must complete the name changing application form. Then, the department will observe and investigate before considering the application for endorsement letter approved by Penang S.S.Mufti. This letter will be issued immediately to  the Nasional Registration Department of Malaysia for futher action.
